  • We require all patients to have a valid credit card on file, which will be charged at the time of visit or service rendered unless payment is provided with cash or checks at the time of the visit. Returned checks will incur a fee.

  • We request at least 24 hours' notice if you need to cancel or reschedule an appointment. Failure to provide notice may result in a missed appointment fee. Multiple missed appointments or excessive cancellations may lead to the termination of your patient relationship with our practice.
